London firm fined $125,000 in 2020 M’Chigeeng fatality

GORE BAY—A London-based company, Integrated Solutions Inc., was handed down a fine of $100,000 following a guilty plea in the fatal injury of a worker at a M’Chigeeng residence in 2020 while installing a satellite dish—the fine was subject to a 25 percent victim fine surcharge bringing the total levied against the company to $125,000. 

The plea was entered before Justice of the Peace Lori-Ann Toulouse at the Provincial Offenses Court in Gore Bay and the company was convicted on July 29, 2024.

According to the facts of the case, a worker had been installing a satellite dish near the edge of the roof of a M’Chigeeng residence. The company pleaded guilty to “not ensuring the worker used fall protection while on the roof and failing to take every precaution reasonable in the circumstances for the protection of the worker” as required under the provisions of the Occupational Health and Safety Act.

The deceased worker was contracted by Integrated Solutions to install satellite dishes and was working near the bottom of a sloped roof on October 21, 2020, when they fell and suffered fatal injuries.

Charges were laid following an investigation by the Ontario Ministry of Labour, Immigration, Training and Skills Development.

The victim surcharge was levied under the Provincial Offences Act and is credited to a provincial government fund aimed at assisting victims of crime.
