Home and community care services are now just one quick click away

NORTHERN ONTARIO—Help is only a click away thanks to a new website—www.northeastcss.ca—that will connect Northerners to the home and community care services they need to stay healthy and independent at home.

The website was developed with the support of the North East Local Health Integration Network (NE LHIN) by Healthline, a not-for-profit organization that provides reliable and up-to-date online information about health services to people across the province. Users can also access this new site by clicking a button located on the Healthline at www.northeasthealthline.ca. 

The site is available for use by Northerners looking for services for themselves or a loved one, as well as health care providers such as physicians, nurse practitioners, and nurses looking for support services for their patients.

“While engaging with thousands of Northerners over the past year alone, we heard first-hand how people often have trouble finding the support services they need to stay at home–services like meal delivery, transportation, adult day programs, assistive devices, palliative care and caregiver respite,” said Louise Paquette, CEO of the North East LHIN. “The North East LHIN is pleased to partner on this new website to help increase awareness of what’s available to Northerners in their communities.”

The website has been embraced by the North East Regional Community Support Services Network which provided input to shape its creation. This network is supported by the North East LHIN.

“This website is one of the projects our regional network has undertaken to help clients and health professionals,” said Marie Leon, chair of the North East Regional Community Support Services Network. “We’ve also developed other tools, such as a video and booklet to create awareness of services and where to go to find help.”

In addition to exploring what’s available online, Northerners can also use a newly developed common referral form to either self-refer or refer others to the services delivered by North East LHIN-funded home and community providers.
